Recuperative Care Criteria

Admission Criteria

We look forward to Housing, Healing, and Helping your patients. Please see a list of our frameworks below. We are always willing to make special arrangements to support you and your patients. Just let us know how we can assist.

To refer a patient to our recuperative care, they must meet the following eligibility guidelines and criteria:

  • Able to independently complete all Activities of Daily Living (ADLs). Wheelchairs and/or DME devices may be accepted if the patient has:
  • The ability to use the DME device safely and understand its proper use and handling (e.g., transferring from toilet to wheelchair).
  • The ability to use the DME without assistance (no caregiver assistance required).
  • The ability to ambulate at least 20 feet before hospital or facility discharge (with or without DME).
  • Able to self-administer medication with staff oversight.
  • Continent of both bladder and bowels (if diapers are used, the patient must change and maintain hygiene independently).
  • Medically and psychiatrically stable at discharge.
  • Alert and oriented (aware of name, place, date, and situation).
  • Not a danger to self or others (not suicidal or violent).

If specialized care or equipment is required, please discuss this with our staff before referral (e.g., CPAP, special bed, specific wound care requirements, detoxing).

Please review these criteria with the treatment team and the patient before making a referral.
