What is a Community Health Worker?

A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has a close understanding of the community they serve.

The Community Health Workers (CHWs) and Peer Support Specialists at the RIS Foundation are extensively trained in health education and resource assistance. During your stay at the RIS Foundation, and even after you leave, your assigned community health worker or peer support specialist will be your dedicated advocate.

These professionals will serve as a vital link between you and various health care and social services, helping to bridge any gaps in understanding, communication, and accessibility. They will assist in monitoring your health condition, conduct basic health screenings, and provide essential follow-up care. Additionally, they will offer informal counseling to you and your family members who may be dealing with health issues.

Their goal is to ensure you receive comprehensive support and guidance, facilitating your journey towards better health and well-being.
